Part-II
■ Contribution :
Each subscriber may pay up to Rs.300/- (Three Hundred) per annum. The payments may be made monthly @ Rs.25/- per month or as may be convenient to the subscriber in multiples of Rs.25/-. The State Government will make matching contribution. The interest on the balance at the credit of a subscriber will also be paid by the State Government at a rate fixed by the State Government. If a subscriber pays less than Rs.300/- in a year, the State Government's contribution will be equal to the multiple of Rs.30/- per month as subscriber paid (i.e. a subscriber pays Rs.180/- in a year, then the State Government's contribution will be the Rs.180/-) not more than Rs.360/- in a year. A subscriber cannot make contribution more than Rs.300/- a year.All payments are to be made within the financial year and there will be no arrear payments.
● The account of a subscriber will be automatically closed if does not make any contribution continuously for three financial years.
● The total amount lying in a subscriber's credit will be paid to him/her on attaining the age of 60 years.
● In case of premature termination on account of death, the total amount lying in the account of the subscriber will be paid to the subscriber's nominee on receipt on an application in Form-VIII along with necessary certificates enclosed with the form.
● If a subscriber - worker does not want to continue as a subscriber under the Scheme, he / she may be given final payment of the total balance lying at his/her credit ( that is the worker's own contribution, matching contribution of the Govt. and the interest) after expiry of the lock-in period of three years from the date of his/her enrolment,  with the approval of the concerned Advisory Committee on receipt of an application in form-IX from him/her.
▶ At the time of enrollment a worker under the Scheme will be issued an Identity Card-cum-Pass Book in Form No-II under the signature of the authorized officer. The Minimum Wages Inspector / Assistant Labour Commissioner attached to the Labour Department are authorized to issue Identity Card-cum-Pass Book.
The contribution will become due on and from the next month following the month in which the Identity Card-cum-Pass Book is issued.
